Alternatively, you can complete an appointment request form online or in person at our center. For urgent matters, we offer same-day appointments based on availability.

Many of our services are provided at no cost to eligible individuals and families. Some specialized programs may have fees based on a sliding scale according to income and ability to pay. We accept most insurance plans, including Medicaid and Medicare. Our staff will work with you to determine eligibility for free or reduced-cost services and can assist with payment arrangements if needed.

For your first appointment, please bring a valid photo ID, insurance card (if applicable), any relevant medical or mental health records, a list of current medications, and any referral documentation if you were referred by another provider.
